This position is located in the Mental Health Service, Homeless Programs.
The Justice Outreach Social Worker is a Senior Social Worker whose primary responsibilities are to provide psychosocial assessments, case management services, continuous quality improvement activities, evaluation/consultation, placement services and referral and linkage to various community agencies for Veterans who are within 6 months of release from correctional institutions and/or are being monitored by the courts, including treatment or problem solving courts. The Justice Outreach Social Worker is administratively responsible for clinical programming and daily operation of the program, as well as the management of program data.
Justice outreach activity will focus upon justice-involved Veterans in one of three circumstances: 1) at the front end of the justice system (jails or courts) (VJO-Veterans Justice Outreach), 2) upon prison reentry (HCRV-Healthcare for Reentry Veterans) or 3) a hybrid model encompassing jail, court and prison outreach (hybrid VJO/HCRV).
Duties include (but are not limited to):
Provides direct clinical services to Veterans that are involved in the criminal justice system and/or currently residing in jail or prison.
Provides outreach to justice involved Veterans in a variety of settings outside of the VA clinics; the settings include but are not limited to: jails, shelters, soup kitchens, non-profit agencies, and street outreach which includes places not meant for human habitation like camp sites and parks.
Conducts initial assessments, develops care plans, and provides case management to link Veterans with necessary resources. Case management duties include care planning, care coordinator for VA and community services, and documentation/reporting duties for services offered and progress obtained.
Completes documentation in the computerized Veterans record and national reporting databases for the program.
Work requires visits in the community. Incumbent must drive and have a valid drivers license.
Work Schedule: Monday - Friday 8:00 a.m. - 4:30 p.m. and may include some evening and weekend hours for outreach activities and care coordination
Duty location for this position will be located at the VA Community Resource and Referral Center at 819 Dorcas Street in Omaha

GRADE DETERMINATIONS:
Social Worker Program Coordinators and Manager. For Social Worker Program Coordinators, grades are determined by the scope of responsibility and the type, size, and complexity of the assignment.(a) Experience, Education and Licensure. Individuals assigned as social worker program coordinator or manager must be licensed or certified at the advanced practice level. Their experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. They may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. In addition, the candidate must demonstrate the professional KSAs in subparagraph (b) below.(b) Demonstrated Knowledge, Skills, and Abilities
1. In-depth knowledge of the program coordinated, and demonstrated knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
2. Knowledge and skill in management/administration, which includes supervision, consultation, negotiation, and monitoring.
3. Ability to supervise multidisciplinary staff assigned to the program.
4. Ability to organize work, set priorities, meet multiple deadlines, and evaluate assigned program area(s).
5. Ability to provide training, orientation, consultation and guidance within clinical specialization of practice.(c) Assignments. The KSAs shown above apply to all Social Worker Program Coordinator grade levels.GS-12 Social Worker Program Coordinators. Program coordinators at the GS-12 grade are administratively responsible for a clinical program providing treatment to patients in a major specialty, such as Spinal Cord Injury (SCI) coordinators, Visual Impairment Services Team (VIST) coordinators, Homeless Veterans Program (HVP) coordinators, Community Nursing Home Program (CNHP) coordinators. They may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. They manage the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget.

PHYSICAL REQUIREMENTS: May require prolonged standing (up to 3 hours per day), moderate walking (up to 3 hours per day) and manual dexterity. Requires carrying light items up to 15 pounds such as books, files, laptop computer, etc. Time will be predominately spent with face-to-face interactions as well as making referrals, using the phone, using the computer, faxing, facilitating groups and individual contacts. Extensive community work is involved with this position and requires work in various locations including homes, shelters and places not meant for human habitation. The incumbent will be required to work in various weather elements and be exposed to situations outside of supervision in which the incumbent will follow the Community Safety Standards outlined by VA NWIHCS. The incumbent may be exposed to Veterans with infections and disease related conditions and may required to don protective clothing in isolation situations. Incumbent must drive and have a valid drivers license and meet the VA driving requirements outlined in accordance with VA Handbook 5019, Part IV, Appendix B.
